How do I become a Live Well Logan provider?

Are you interested in becoming a Live Well Logan provider?

The next round of applications will open on Monday, 11 January 2021. These applications will be for the 1 July 2021 to 30 June 2022 program.  Applications will close on Sunday, 14 February 2021.  To apply to become a provider, please visit SmartyGrants Live Well Logan Request for Proposal application form.

If you would like to find out more about what you need to become a Live Well Logan provider, we’d love for you to attend our Interested Providers Information Session.  This session will give you an overview of the Live Well Logan program, what it means to be a Live Well Logan provider, as well as information on the application process.

The Interested Providers Information Session will take place online by Microsoft Teams on Wednesday, 20 January from 10.00am to 11.30am.  Please RSVP by emailing with your name, organisation name and phone number by Monday, 18 January 2021.  If you’re unable to make it, please email us for a copy of the presentation and more information from the session.

How does it work?

We will be looking to deliver activities across all areas of our city. Activities should:

  • be good value for money
  • meet existing or emerging community needs
  • show regular participation and positive feedback (if delivered before)
  • be held in line with our terms and conditions.

We will give preference to providers based in Logan. If you’re looking for a venue for your activity, try one of our fantastic community centres or parks.

Benefits of participating

As part of the Live Well Logan program, you may receive:

  • A service provider rate of $20 per hour. You can also charge up to $5 per participant, per session (which you will retain).
  • A travel subsidy for eligible applications of $20 per activity.  To be eligible, you must deliver activities in our regional suburbs (and not live in the area). These include Flagstone, Greenbank, Jimboomba, Mundoolun and Yarrabilba.
  • Reimbursement of a part of your venue hire. Cooking activities may also have part of the cost of ingredients reimbursed.
  • Promotion of your activity through our booklets, website, social media and community events.

Terms and conditions

As a provider, you will need to give us evidence of:

  • $20m public liability insurance
  • current Blue Cards (if required by Blue Card Services)
  • current first aid and CPR qualifications
  • a risk management plan
  • a copy of your COVID-Safe Plan for each activity.  As a business, it is your responsibility to make sure your activities are COVID-safe.  If you need advice call the COVID hotline on 13 42 68.
  • relevant qualifications for your activity (if required).

If approved as a provider, you will also need to:

  • sign a contract with Council
  • attend an annual provider forum, even if you have attended before
  • book your venue and arrange payment for the hire costs, we will reimburse you for this after your activity is delivered
  • notify Council of your event if you want to use a Council park
  • take bookings (if required) and manage participants enquiries
  • ensure every participant fills in a registration form at the beginning of each program block
  • provide us with the completed registration forms along with your invoice at the end of each block
  • complete end-of-term reporting by the due date  
  • promote your activity - all promotional material must include Live Well Logan and Council logos
  • update us with your new certification and qualifications as they expire
  • wear a Live Well Logan t-shirt and display a corflute sign at every activity (we will provide these).

Invoicing and reporting

Each Live Well Logan activity runs in three month blocks. At the end of each block, you will be able to lodge your invoice along with your registration forms and reporting.

It's our policy to pay invoices within 14 days of the invoice date. We cannot pay until you've given us all your reporting and forms, so get those to us as quick as you can at the completion of the block!

There will be a 4 week break for all activities between Monday 13 December 2021 and Sunday 9 January 2022.
